Digital Intake Forms
We understand that filling out paperwork can be time-consuming and inconvenient. That’s why we offer digital intake forms that you can complete from the comfort of your own home or office.
These forms are secure and easy to use, and they allow us to gather all the information we need to provide you with the best possible care. Once you book an appointment, we send you an appointment confirmation email with a link to complete your intake forms. To access the digital intake forms, simply click the link, login to your account and click on the “Intake Forms” link to fill it.
Cancellation Policy
We understand that unexpected events can arise that may require you to cancel or reschedule your appointment. However, we also value the time of our healthcare providers and staff, as well as the time of other patients who may be waiting for an appointment. That’s why we have a clear cancellation policy in place to ensure that everyone’s time is respected.
If you need to cancel or reschedule your appointment, we ask that you provide at least 24 hours’ notice. This allows us to offer the appointment time to another patient who may be waiting for care. If you cancel with less than 24 hours’ notice or fail to show up for your appointment, you may be charged a cancellation fee.
We understand that emergencies and unforeseen circumstances can arise, and we will do our best to accommodate your needs whenever possible. However, we ask that you please be mindful of our cancellation policy and provide as much notice as possible if you need to cancel or reschedule your appointment.
